Provide as much detail as possible about the requested records. If you need more space than provided for in the online form, please use a separate sheet of paper and mail it in with your application fee.
Specify the time period for the records as precisely as possible (e.g., from 07/21/2008 to 11/30/2009).
If you are requesting a correction of your own personal information, describe the correction you want and provide any supporting documents. If possible, provide copies of the information to be corrected and the information you wish to have it replaced with. Mail the supporting documents and any copies of information to the address provided below.
Select whether you want to receive copies of the records or examine the original (which may only be done on site).
